Maybe instead, it's a hereditary condition making them change sex during puberty. A young boy will starting becoming more and more feminine as they get older, getting softer features, a female frame as their curves pull in and hips grow out. Starting at around 13-14 his breasts start growing and by the time he is finished with growing around 18, he will be a fully functional adult female ending with the manhood reshaping into a woman hood though out as it gets small and looks more and more like a woman's privates.
Maybe it is just one person each generation, or all of them or somewhere between, just being randomly inherited from carriers and suffers.
