Option 1:
During a mission Tony is hit with a strange ray that slowly starts to deteriorate his bladder and bowel control, putting him back in diapers and potentially out of the WOOHP.

Option 2:
Tony can be reckless sometimes, and he's prone to getting a big ego, his siblings get sick of his attitude problem and plan to teach him exactly who the baby brother is.
