What about Willow and Xander's Love interests for much of the latter half of the series, Tara Mclay and Anya Jenkins?

Tara is soft and gentle and a careful witch, I doubt she'd deliberately or accidentally do this to herself, so who screwed up?

Bold, blunt, and pretty much a nymphomaniac, Anya is a lot more likely to do this to herself, and she does own a magic shop, containing all sorts of things.
