As I suggested before, Fred on a few occasions is implied to have at least some disappointment at her small bust (particularly since she spends most of her time around Cordelia who... yeah), perhaps she attempts a magical solution to her woe, only to accidentally end up sprouting more breasts.
Alternatively, before joining Angel Investigations, Fred was exiled to the dimension of Pylea, home of Lorne, where Humans are used as cattle (women are explicitly referred to as cows) perhaps... that's a little more literal; one of the larger divisions of the Multibreast fetish is cow girls; usually with 4 big milky breasts, and possibly other stuff like udders, horns tails... Maybe Pyleans use some kind of potion to make transform women, alternatively, entering Pylea temporarily altered Angel, making his vamped out form more wild, while affording him the ability to walk in sunlight and see himself in mirrors in 'human' form, perhaps simply being in Pylea causes women to transform in the way we're describing, and of course, just because Pylea's effect on Angel only lasted while he was in Pylea, doesn't mean the same rules apply to Fred, or Cordy...
