1) The crew's next delivery involves traveling through a nasty wormhole. They are warned quite extensively to wear protective equipment going through, but Fry wasn't paying attention. When it comes time, he fails to protect himself and winds up shrunken on Leela's captain's chair.
2) Fry goes exploring through the Planet Express building while he kills time before a delivery. When he stumbles upon a machine, he can't help but play with it. Unfortunately, the machine is a prototype machine Amy made as a project. He picks it up and plays with it, and it immediately shoots him and shrinks him. When Amy stumbles upon him, she decides to run some tests to see if her machine worked correctly.
3) Takes place during Season 2 episode 11 “How Hermes Requisitioned his Beat Back,” while Morgan Proctor is Planet Express's bureaucrat. When Fry gets upset at her for resetting bender, she uses her (legally acquired) shrink ray to cut him down to size. Now he has to deal with a giantess that is turned on by him.
4) When a new delivery takes the crew back to Amazonia, they are very careful not to get on the local people's bad side. That is, until Fry clumsily breaks a priceless artifact, which shrinks him in the process. The amazonians take it upon themselves to punish Fry using their bodies.
